If you are planning on going out of town and leaving your cats under the care of a pet or house sitter, filling out this form will enable us to properly care for your cat in the instance of him/her becoming sick and needing medical attention while you are away.

This form is designed for those who have a scheduled procedure with the Cat Clinic of Fernandina which will require anesthesia (all surgeries, lion cuts that require sedation, and other procedures). Please do not fill out your pet's weight (we will check and update this information upon your pet's arrival) or any part of the table at the bottom. Also, be sure to give us an accurate phone number at which you will be reachable when you are away in the case that we would need to contact you.

Having problem with a cat that is spraying or defecating outside of the litter box? Please fill out this form in its entirety and bring it with you at the time of your appointment or consult. The answers are very important as they will help Dr. Mersereau to better understand exactly what is going on and what steps we can take to correct the behavior.
