If you are planning on going out of town and leaving your cats under the care of a pet or house sitter, filling out this form will enable us to properly care for your cat in the instance of him/her becoming sick and needing medical attention while you are away.

This form is designed for those who have a scheduled procedure with the Cat Clinic of Fernandina which will require anesthesia (all surgeries, lion cuts that require sedation, and other procedures).
